How to Create Note Templates
Use the guide below to build and personalize your own note template, so your notes are structured exactly how your practice prefers and automatically generated after each session.
Have existing templates? Email us your templates, and our team will help pre-load them for you.
1. Open Template Page
- Click Scribe from the left navigation
- Select Templates
- On the Templates page, click Create Template (top right)
2. Choose Template Type
You can either:
- Select a pre-loaded template from the Note Format dropdown
- Or create a fully custom template
Enter a Template Name before continuing.
3. Sections & AI Instructions
This is where you define the structure and content of your generated notes.
Add and Configure Sections
- Enter a Section Name (e.g., Chief Complaint, Treatment Plan, Exam Findings)
- Click the + button to add the section
- The Section Details panel will appear on the right
In AI Instructions, provide clear guidance on:
- What information should be extracted from the session
- How the section should be formatted
- Any required tone or structure
- Examples (optional but recommended)
- Preset options the AI should choose from
Example AI Instruction
If your section is Treatment Plan, you might write:
Clear, specific instructions produce the best results.
Important: Please save your progress incrementally by scrolling to the bottom of the page and clicking Save Changes.
4. Formatting Options
These settings control how the final note appears when generated or exported.
Section Headers
- Recommended for referral letters, treatment summaries, or patient-facing documents.
- When enabled, section titles will appear in the generated note and exported files (PDF/DOCX).
Include Signature
- Adds the provider’s name and signature at the bottom of the note.
- Recommended for referral letters and formal documents.
Auto-Generate Notes
- When enabled, this template will automatically generate after every recording session.
5. Save Template
After completing setup:
- Review your sections and formatting
- Click Save Changes
Your template is now ready to use.
